Not taking the time to research drugs. Seriously guys. Especially in medical shows/etc. Like it's fine if it's a layman confused by drug names/families, but when I hear, "10cc of benzodiazepine STAT!" I roll my eyes so hard they nearly fall out.

God, same. But only because I work in a hospital pharmacy and actually buy the drugs for said pharmacy.
