It only explains your best friend's situation if it's caused by a genetic disorder that both parents carried the genes for, rather than something else (there are a lot of epigenetic factors that can affect fetal deveolpment) - and even then, only in that close interbreeding significantly raises the statistical likelihood of offspring inheriting genetic traits carried along the family line. And with a single generation of first-cousin breeding, it's really just bad luck if a deleterious trait shows up - most of the major examples of Why Inbreeding Is Bad are the result of close-kin breeding followed by close-kin breeding followed by close-kin breeding for multiple generations.
