Oh, also, Australia. I will add a caveat that my mother was from a country town and grew up in the 50s, so I have secondhand information from an older generation.

I will also add that such issue IRL are totally different to such issues in fanfic. Fanfic can do almost anything and make it work out good, because it's fiction.
