In what fics? I've only ever seen this in fics where both characters are obviously cisgender. Things like "lubed up his cunt" or "played with his balls while he fingered his pussy" (I've even seen the term "manpussy" once lol), all the while there's zero sissy/feminization kink going on, etc.
