In middle school, some parents successfully ousted the school head on the grounds that he was yelling and harassing students he didn't like. Some parents agreed with this, others thought he was targeted by parents who were angry that their kids hadn't gotten high school recommendation letters, and the teachers were caught in the middle of the feud. Many teachers quit to escape the drama, and to put it bluntly, their replacements were completely unqualified to teach. It was a mess.

In high school, a teacher I never met hid a video camera in the ceiling of the girls' bathroom.

Also in high school, a PE teacher who berated and looked down on his students was supervising the climbing wall. A student came to him and told him that she wasn't sure her harness was set up right, but he didn't look at it and told her to just climb. Turns out she'd accidentally put the rope through a little side loop that was meant to hold a water bottle and wasn't meant to hold the full weight of a climber. She survived the fall, but I don't know if she ever fully healed. He was fired.
