I have OCD too, and I feel this really hard. It doesn't happen to me with fic (at least not as far as I can recall), but sometimes someone will tell a really harmless joke that nonetheless hits me just the right way to really upset me for a long time. (That happened tonight, actually; ugh.) You might already know this--you probably do, I guess--but intrusive thoughts are often a real issue with OCD. Random offhand upsetting thoughts or mental images get snagged in your head and you imbue them with meaning and agonize over them. Sometimes, like the other commenter said, they're sexual. Say you're suddenly hit with the thought of sleeping with someone you never would/shouldn't for some huge taboo reason; a non-OCD person would probably just think "ew" then move on, but someone with it might think "oh shit, does that mean I really want to fuck ___?? What kind of MONSTER am I???" etc.
