Generally a combo of canon age, how the canon treats them, and how the fanwork treats them.

Like in Steven Universe, Connie's a kid, the canon treats her like a kid, so unless the fanwork is aging her up and making it very clear she's AU an adult? She's a child.

Sapphire is an adult, the show treats her like an adult, so even though she's 'child coded' unless the fanwork is very obviously treating her as a child I'm going to see her as an adult.
