I haven't had the person leave someone else for me, but I've been in a similar situation. I wanted to stay friends but every time I responded to this person I would worry that they were interpreting me being nice as me being interested in them back, which they admitted they would try to twist these things around in their head to keep their hopes up, and then I'd feel bad if I got harsh or blunt.

THIS. I've had this experience - to varying degrees of intensity - with several guys I've been friends with over the years. One of the first times it happened, it kind of messed me up for a while. I was pretty young, and the whole situation just went on and on, and I started to feel like shit about myself as though I was doing something to cause it, but at the same time I developed all this buried resentment for my friend for not just dealing with his own shit. So yeah, definitely not fun.

But at the same time, I still love the tropes OP is talking about. I guess for me, my own shitty experiences with unrequited feelings between friends doesn't have much bearing on how I feel about those tropes in fanfic, because in fanfic it's pretty much never one person trying to persuade the other person to love them back, and behaving weird and sad and erratic when it doesn't happen. It's almost always that they've both loved each other all along but mistakenly think their feelings are unreciprocated.
