I haven't watched it but from what I heard, Sherlock was forced to say I love you and get Molly to repeat it back as some kind of twisted game the antagonist was making Sherlock play.

Eurus (the sister) claimed to have wired Molly's flat with explosives. The clue was a coffin that was Molly's size and the words of "I Love You" on the lid. Basically, to get Eurus to turn off the explosives, Sherlock had to convince Molly to say "I Love You" to him.

Molly turned it around and told him to tell it to her like he meant it first.

He ended up saying it twice. Once was rather flat. The second was... not.

Molly said it in return.

Then Eurus cut the contact and told Sherlock there hadn't been any bombs at all. Sherlock frankly went ballistic and destroyed the coffin with his bare hands.

Later, in the end scene, you do see Molly entering the flat looking quite happy. And that was all the closure we got.

Sherlolly shippers are over the moon. Watch the scenes, interpret as you wish basically.
