IIRC, at some point JJ gets kidnapped and tortured by a former (admittedly skeevy) colleague she knew back when she was doing some weird special ops stuff in the Middle East, who tries to force her to give up security codes to some high security database. He tasers her stomach, causing her to miscarry what would've been her second child, and threatens to rape her. Eventually she breaks free, but in this episode it's the one year anniversary of that kidnapping/torture, and she confronts her kidnapper in this imaginary scene of catharsis. It's kind of cheesey tbh, where the bad guy is all but twirling his mustache and going BWAHAAHHAHA, etc.

Thanks for sharing. That sounds very unpleasant, I can see how there's a lot of ways a heavy scene like that could go wrong.
