I've heard it directly from some Japanese BL fans that they self-insert as the smaller/daintier of the couple. It makes them feel more empowered to see themselves as a man, because they have more power/opportunities, but they also want to be loved by men (since they're straight).
So it's not homophobia, it's sexism at the heart of it. Which is still rampant in Japan.

This explanation makes a lot of sense to me. And I think it's probably the case for many/most BL fans outside Japan, too.
