Are there any OFFICIALLY translated R18 otome/BL games? Or otome/BL games in general? I know there are a few with fan-translations, but I wonder if there's a proven market for them in the West to justify official translations.

Are you asking if there are any otome releases in the West? Because yes, there are, and they've done well. Hakuouki is a good example. There's a good number available on Steam as well.

Hakuouki (and Demon Bond) don't have sex, but are massively good.

Voltage gets a little spicy, but I tend to dislike their heroes, so. (Bastard billionaire boyfriends aren't my kink.)

For otome games, the answer is no. There are a few Western otome 18+ games that I know of, but no official translations of Japanese R18 games. You might be able to find some fan translationed of demos, but that's it.

The otome market in the West is actually starting to grow, I know of at least four more titles coming out this year. Sex-wise, the only one I've played that has vaguely sexual content is Ozmafia!!, which has like ten cut-to-black sex scenes. Hakuouki also has one of those, but I'm pretty sure Ozmafia!! is the closest thing the West has gotten to actual sex so far. Wait, no, there are the mobile games, but they're all bullshit freemium where you have to pay extra for the good stuff, so they barely count.

First 18+ PC otome game official release (I believe). Currently fully translated and edited, but not released yet! Keep an eye out for the programming and beta testing to finish. And even more relevant to OP, one of the boys is a masochist (figured if you used Utsuro in the secret, it might be your thing).
