Many years ago I had a situation similar to this except I was the writer. I sent the beta my doc and she sent me back a wonderfully gushy email with a bunch of things she liked and/or though needed changing. Because I had never in my life used "track changes" in a document (and mine was off by default) I didn't realize or see that she had annotated many many things in the document that needed fixing. Prior to posting the fic I changed the things she mentioned in her email but flat out never changed anything any of her annotations because I didn't see them. The situation got really ugly for me with one of the beta's friends and though the fic itself was really popular, it became a dead WIP because every time I open it up all I remember is the vitriol spewed at me over the beta and I have no inspiration to finish it.

So not to excuse the writer in the OP's situation, but sometimes there are really innocent reasons why something happens.
