Cate Blanchett will always be Galadriel to me (though that may change after Thor 3).

Yes, Terri Hatcher will always be Lois Lane both in that's how I remember her and in that she's my favorite Lois.

Honestly, most of the LOTR cast will always be those roles to me. Elijah will always be Frodo, so I can't really watch him in anything else. Viggo will always be Aragorn, and I find it strange to see him in other things. Sean Bean and Ian MkKellin are the only two that I can see in other things without being thrown out.

Craig Parker will always be Darken Rahl. Seeing him as a blonde elf in LOTR is weird sometimes because of this. But damn is he fine as Rahl. The look very much suited him.

Maggie Smith is a weird case because she'll always be McGonagall to me as she looks now, but I can watch her in earlier stuff without being weirded out. I love her in Clash of the Titans, for example.

Robert Picardo will always be the Star Trek holographic Doctor. It was always strange seeing him on Stargate as a less nice character because of this.

Walter Koenig will now always be Bester to me. Which makes seeing him as the heroic Chekov very strange.

This one is so weird for me. I saw him as Chekov first, funny and boastful and innocent, so it was so weird to then see him as Bester, all soft-spoken and sinister and creepy as hell. But he was so fabulous as Bester. He's honestly amazing to watch. So I think I probably do think of him as Bester first these days as well, it just always jars me a little when I realise it.

I saw him as Chekov first too. But Babylon 5 is probably my favorite tv show. And he does such an amazing job at being a villain. And yup, a bit jarring when seeing him as Chekov now that I've seen him play a bad guy.
