contacting an acquaintance from forum you ditched two years ago

We were sort of friends but not quite and I ended completely leaving the forum without a word because of reasons unrelated to her. I found her tumblr and want to reconnect, but I have no idea what to say. Any ideas or should I even bother?

Just a basic, "Hello, don't know if you remember me but I'm [name] from [old forum] and just wanted to say hi and I hope all is well with you!" would work fine.
