They are a couple - I don't think you can seriously watch the series without it being obvious. But at the same time, it's not completely explicit, and the creators are unsurprisingly a little coy (in all fairness, all mangaka/anime creators tend to want to leave it to the audience to interpret absolutely anything). But I think you'd have to be pretty delusional or ignorant to watch it without realizing the show is about their romantic relationship. There's a lot of erotic insinuation especially in Yuri's skating for Victor.

Fat shaming... that one's a little tougher. Yuri has gained weight in the first episode or two, much to the outrage of anyone invested in his skating (since he can't really be a pro skater if he isn't in shape, like with most athletes). So his nickname is "piglet", and there's some unflattering joking shots of his gut and some other characters tease him for it. But there's never an implication that he's unattractive because he gained weight, just that he can't skate professionally. Victor already likes Yuri from the start. He had gained weight as early as the Grand Prix Finals the previous year, when he first met Victor. Victor is a little ruthless about Victor losing weight, but more from a coach standpoint. He's already openly flirting with Yuri from episode 1.
