That one additional step makes Jon/Dany have double the shared genes Jon/Sansa have, which does have a huge impact on risk factors for having children (whereas it's actually perfectly fine for cousins to have children). And that's not counting the fact that Dany is already incredibly inbred through generations of Targaryens marrying sister to brother.

Jon definitely wouldn't want to marry a sister or aunt, but cousin marriages are not only normal in their world, they're common IRL around the world.
