If you're posting to Ao3, in addition to tagging and rating appropriately, you could archive-lock it? I mean, it doesn't guarantee that underage people will come across it, but it's an additional security.

My advice is write it without any fandom-specific elements. Change the names. Publish it as an original story (or erotica). Only you will know. No one under age will read it or complain. You might make a few buck, too. Threesome stuff is popular right now, I think. (I'm not the target audience but somebody is!!)
