I don't think it's any more awkward than kitchens that place microwaves up high, like over the stove vent. You're less likely to pour hot soup all over yourself lifting a bowl up from down low than you are bringing one down from above your head. At my grad school apartment, I had to put the microwave on top of the (full height) fridge and stand on a chair to stir things.

The real question is: why aren't more luxury kitchens designed with a microwave at a more convenient height (i.e. counter or chest height)? Microwaves aren't random add-on appliances anymore. When my parents remodeled their kitchen, the microwave went at counter height, and they aren't rich celebrities.

Omg, microwaves over the hood! I know i would pour boiling liquid on myself or something. They are just nuts. An under-the-cabinet one, attached to the bottom of a cabinet, seems the smartest way to go.
