Hear me out--I'm as upset with Casca's current state as the next fan is. I don't think there's a single fan out there who is happy with her state. The characters certainly aren't happy with her state, either. Guts sure isn't, and, as shown in one of the later chapters, neither is Farnese. In fact, I've seen many fans state that they hoped she'd be killed off altogether because they're tired of putting up with her mentally crippled state. I know I at first thought it would have been better for her to be killed off during the Eclipse because I couldn't stand seeing her like this.
Miura could have chosen to gone down the route the fans wanted--he could have had Casca raped and then killed before Guts' eyes. It wouldn't have changed Guts' current motivation for revenge on Griffith, and the manga probably would have ended sooner. Casca's role would be sealed as the love interest who was unfortunately shoved into the refrigerator.
This is not the case. The reason Casca hasn't been killed off at this is because she's still vital to the plot and the characters in ways that stretch beyond Guts' bloodlust. She aids Farnese in her development as a braver, stronger person. Her state doesn't prevent her from showing emotions, humanity, or a personality. She's still very much a character in the story. Above all, her getting cured is the prime objective of the group.
The reason Guts wasn't raped and Casca was runs deeper than her simply being a woman. It has everything to do with her character. She knew Griffith longer than Guts had. She saw Griffith as more than just a leader of the pack--she saw him as a father, a brother, possibly even a lover in her eyes. There are hints that her love for Griffith might even run deeper than her love for Guts. The manga foreshadows this in one particular chapter when she's in her current state (IIRC, the line was "What you [Guts] want may not be what she [Casca] wants.") I wouldn't be the least bit surprised if it becomes a conflict in the future after she's cured. Griffith/Femto knows this. He knew this as he raped her and as she consented to him with the last of her strength, just after telling Guts not to watch. God knows how many conflicting feelings were stirring inside her at this point, hence her blocking out every single event in her mind altogether.
And then there's the child she gave birth to after the Eclipse that was corrupted by Femto.
