Oh hush, for all you know maybe the author IS getting help and dark doujin/art is an outlet. Or maybe they're perfectly fine and just have a vivid imagination??? Do you think every horror writer needs help? Or everyone who writes a crime novel?

what is the purpose of a doujinshi when compared to a horror movie? a doujinshi is a self-serving fantasy, so there's more than a significant difference.

people who write torture porn ala hostel and saw do indeed also need help.

Okay I don't even like torture porn but you're really losing me with the "the writers of Saw need help" angle. Like. 1. Does everyone involved in the now 8 Saw movies also need help? Everyone who paid money to see them? This is a wildly broad brush you're painting with.
2. Can you really not see how people can enjoy things they don't like irl... even in doujin...
3. Most fiction is at its core, self serving fantasy in one way or the other
