I disagree with a lot of the content policing that goes on in fandom. But it's honestly strawman-ing things a fair bit to claim people "can't tell fantasy from reality." Most of the people who object to one form of content or another are perfectly clear on what's reality and what's fiction. They just believe that fiction can influence reality, and therefore they do not view fiction as inherently harmless.

Except they feel it's perfectly ok to harass and harm real people because of the idea that fiction might influence reality. That's why people say they can't tell fiction from reality. They're ok with real violence to stop fictional content.
