Legit Reasons-
1. Sam knows that Dean feels that its his responsibility to take care of Sam and he knows that Dean needs to take care of Sam to feel that he (Dean) is important and he gives that to Dean.
2. My Sam is too much of a control freak, and lets Dean top him because its a way of surrendering control and letting Dean take the wheel.
3. Sam is used to Dean taking care of him, and Dean's knee-jerk reacting is to take care of him, which is what My Dean associates with fucking Sam's brains out.
