I don't warn, either. I refuse to do it. If you need warnings, don't read my fic. It's that simple.

I take a lot of care to write summaries that accurately reflect the kind of story I'm writing and the overall tone of the fic, without giving everything away. Given the characters I write aboutand their stories in canon, any reasonably intelligent reader will read the summary and understand that yes, there will probably be violence and somebody might be raped, or endure torture, or even die. It's obvious I'm not writing babyfics. Anyone with half a brain who is worried about "triggery" stuff will move on to something else.

See, this I agree with--it's the attitude that people shouldn't be reading at all, anything, if they can't handle dark stuff that I think is ridiculous. I was trying to articulate this better for another post and gave up, but your comment is exactly what I was trying to say (so I'll try and see if it comes across)--if you're creative enough to write a story that incorporates those things (applicable to dark themes as well as simple plot spoilers such as character death), you should be creative enough to write your summary and notes in such a way that hints subtly at the story's overall themes.

The process of writing fanfiction doesn't END once the story is finished--it ends once it's posted, which includes the header and summary. Too many people forget that and just turn off their intelligence/creativity by the time they get there.

If you don't put any keywords into your summary then how will I find the death/rape/fish fic that I'm after when I do a search?

This. Dammit, I ENJOY RAPEFIC, it's my major fetish, and I hate people who refuse to tag it out of some kind of moral outrage at the PTSDers. I can never find good porn. :(

Now, if most people who wrote fic and didn't warn could do that, I wouldn't object. I use warnings because I can't do that. It's annoying when people are like "LOL it'll spoil the story" and don't hint to things about possible squicks at all.

I think that's what a lot of people mean when they say they want warning, really. That's certainly all I need.

Yes, this. Rating, genre and summary are usually enough for me to get an idea of what to expect from a fic.
