Do you eavesdrop on all his appointments or something? Otherwise, I don't know how you could possible know whether or not he's given these people permission to call him Bob.

No, but on a first appointment for a new person, he comes out to get them from the waiting area and introduces himself as Dr. Smith. I didn't think he would've at some later point told them to call him Bob but I suppose it's possible.

I'm probably putting way too much thought into this, but the title of the thread did say "silly things" lol

Many times, people are more formal when they first meet someone and become less formal over time.

As long as you're putting too much thought into it anyway, consider the possibility that the simplest explanation may be the most likely one. It seems far more likely that, like most doctors, Doc Smith has told his patients that they may call him Bob than it is that your town is full of rude assholes running roughshod over an inexplicably shy doctor.

Of course, it would be easy enough for you to find out. If Dr. Smith is so mild-mannered that he would hesitate to correct a patient about his name, you have nothing to lose by asking him, "Doesn't it bother you when Mrs. Herkimer calls you Bob?"
