It sounds super weird and dehumanizing to me to insist that people in the past probably never "ruminated on their identity". Just because they had different options and contexts doesn't mean that figuring out who you are isn't a pretty universal experience, and gender/sexuality that was outside the norm in a given time and place probably would have provoked some extra consideration from someone living with it.
