Anime like Evangelion and Lain are definitely up there for me.

For films, most of David Lynch's stuff, but Eraserhead and Lost Highway in particular. Coherence, Annihilation, and a downright bizarre British horror called Xtro.

The animated short “Quasi at the Quackadero” that I saw late one summer night as a kid on tv when no one else seemed to be up blew my elementary school mind, even if it’s more oddball than mindfuck. As an adult, another (nsfw) animated short “Malice in Wonderland” comes to mind (warning if you look it up for flashing colors and sudden, loud sound effects).

Oh, Antichrist. I was pretty much on board until the talking fox and my brain went "nnnno thanks." And then it got worse.
