I can understand that tbh

Eh, probably because fic =/= reality. There's lots of things readers enjoy in fanfiction that they don't want any part of in real life. I read a lot of murder mysteries, for example.

And also, fic is idealized, it's fantasy, it doesn't necessarily include the friction and stupidity and frustration of dealing with real people

Exactly. You can have all the elements of a thing that you like, and none of the real life problems.

Nailed it.

Because in fic there's a decent chance it's true love forever and therefore a couples tattoo isn't that shortsighted.

If I was going to get a couples tattoo, one like the secret pic would be cool. Its tasteful and doesn't stand out as obviously being a 'couple' design, so if the breakup isn't so awful you can't bear to be reminded of the relationship you can keep them without it being awkward. And the designs are small and simple enough to be able to cover them easily if you decide you don't want it anymore.

I have such a soft spot for soulmate AU's, most of which involve matching tattoos for whatever reason. So it goes without saying that I agree with you;; But I've read fics where the characters get matching tattoos of their own volition, and it just melts my heart. It makes me see the appeal of a real life equivalent- because there isn't a couple madly in love, about to get married, blah blah blah, who would then say no to a lasting statement of their love on their skin.
But unless I am seriously inked, it just seems corny and regrettable in the long term;;

I'm like this with most things I love for my ships. IRL I don't believe in soulmates, I don't like the idea of being consumed by love, I don't like PDA, matching tattoos, etc, but if a ship of mine? I totally believe my OTPs are soulmates and they can do anything that would IRL annoy me and I'd love it.
