This is pretty much me.

I have such a low self-esteem about my appearance and so much social anxiety that I would never dare ask anybody on a date, I would be suspicious about hidden intentions if somebody asked me out, and even if I said yes I'd probably manage to talk myself into not going because what would be the point?
