Honestly it probably depends on the level of growing up together? I can see why 'childhood sweetheart' relationships where it's implied they pretty much lived in one another's pockets/were always at the other person's house/called their parents mom and dad also could get squicky to people who are squicked by blood-incest.

Same. And I definitely think it's squicky if they were raised like cousins, supposing the families were also close.

I suppose, though "childhood sweethearts" to me already suggests that the relationship is seen as romantic (albeit puppy love romantic) and not as two siblings, so it's still an important distinction.

The Flash TV show had some blurred lines in that regard, with one of the main pairing being fostered with his love interest from about age 8. That was...weird.

Childhood sweethearts calling each other’s parents mom and dad? Never heard of or read about that happening. It has WAY different associations than adopted siblings.
