I wonder if the shame of Catelyn thinking he slept with someone else was easier to handle than the shame of Lyanna possibly choosing to run away with Rhaegar (I know in the show it was lurrrve but I want to believe it was more complicated in the books).

Aerys was always cruel and he was clearly a bomb waiting to explode for any reason, but the spark ended up being Lyanna's 'abduction'. Also directly caused his brother's death, Catelyn's original fiance who she genuinely was into.

Wanting to keep Lyanna unsullied would sort of make sense on why he still kept the secret from his wife, because yeah, if it's about trust I think Catelyn would be motivated to keep the secret since it would put their kids at risk.
