I'd guess it's because A/B/O, alien genitals, and such are one step removed from reality and don't hit quite as close to home.

When I was about to fail out of college (yay, untreated anxiety and depression) fiction dealing with high school students trying to get into college, college students trying to keep up, recently graduated students trying to make it out in the real world, etc. were enough to give me anxiety attacks. However, stories about made-up schools in made-up societies (Hogwarts for example) were perfectly fine.
