It's because people want to cheat and use their paid sick days as extra vacation days. My job also has paid sick leave and yet people come to work sick anyway. They'd rather come to work sick and save those sick days to use as plain old days off, because my job doesn't ask for a doctor's note unless you've been sick several days in a row (which is nice because you don't have to go get a doctor's note if you just have a cold or food poisoning or something like that, but then it also sucks because people abuse it like that). You can just call in sick and they'll accept it, no questions asked.

I get paid sick leave and vacation in the same bucket and sometimes come to work sick if I can’t tell if it’s allergies or not, which is usually the first couple days of any cold, for me. I can have a sore throat and itchy eyes and no other symptoms. And after twelve sick/unplanned days off in a calendar year, I can get in trouble. I get monthly hormonal migraines that can knock me out for a couple days if I don’t take meds in time, so I try to save my sick days for that, puking, snot factory, and fevers. I think I’ve taken one non planned, non sick/pet or relative emergency day in the fifteen years I’ve had this job.

That said, I don’t work in a hospital and once I know it’s something contagious I go home.
