RSAM: Bela makes them look retarded.
No Exit: They destroyed any chance of Jo being a good character with her being an immature amateur.
IKWYDLS: I wanted to know more about Dean's time in Hell, and Sam's little "You won't tell me about Hell, I won't tell you what I did the time you were gone" thing? Asshole, it's HELL. No comparison.
And the dialogue was terrible, Anna was disappointing, Ruby...well she was terrible as it is, this just made it worse. But it wasn't just because Sam decided to hump a demon. We already know Sam's an idiot when it comes to common sense.
Heart: Bad dialogue, the episode felt based around "getting Sam laid" and that was it. Weak writing, although the J's performances were lovely.
The sex scene was hot, but a sex scene does not a good episode make.
But I'm just a stupid crazy fangirl, right?

IKWYDLS: Sam having sex.
Heart: Sam having sex.
I just think it's telling that your worst episode list includes all the episodes where either of the brothers have a sex scene and the episodes where series regular female characters flirt heavily with them.

I hated Bugs too. And Malleus Maleficarum, Wendigo, Ghostfacers...I didn't think it necessary to list them all, just the ones that came to mind.
That scene in Heart was hot as hell.
And if your theory were true, I would have listed DALDOM as a hated episode, where Sam fantasized about Bela? I didn't appreciate it coming out of the blue, but the rest of the episode was amazing.
Conclusions. Stop jumping to them.
