But the design of the game is still going to work to undercut your ability to stay at high humanity. It's not really built around that as a long-term thing.

Sure, but "you're going to really have to work at it" isn't the same as "you can't do it". The monster struggling to cling to their humanity/their self-image of being a decent person is pretty much a stock VtM trope.

Those are the blood mages right? Personally I love playing as good as I can within the forces of evil, though at the same time, I'll purposely make various characters to play each options available.

IIRC Tzimisce are flesh-shapers and the blood mages are Tremere.

Depending on what era you're playing in and what your house rules are, Tzimisce can be both - they were the blood mages before the Tremere became a thing.

I can easily imagine a character concept for this. Perhaps a plastic surgeon who specializes in reconstructive surgery for trauma survivors gets embraced and suddenly is able to do much more for their patients.

Oh, this gives me an amazing idea. Have you ever seen pictures of early skin grafts, in like the 1920s? Because a lot of work was involved in making sure the skin wasn't rejected (and because the work was mostly heavy-duty reconstruction on men who'd been hit by shrapnel) it was pretty ghastly looking while in progress.
Imagine some old Tzimisce sees some of this work, and Embraces the surgeon responsible, under the impression that he's just doing terrifying things to people's faces for his own amusement. Then the surgeon finds he can reconstruct a whole lower jaw in a few hours with no surgical team, and - well, one of them's disappointed, and it's not the newbie!

Isn't that the point of Vampire in the first place? Striving to be a good person despite the insistence of the Beast (and the setting). I'd certainly be interested in a defected Tzimisce at my table, it's a lot more interesting than your typical Sabbat counterpart.
