Off hand, the only times I think he possibly could have killed someone in a way beyond 'general mook collateral damage that he would have no way of directly being aware of' would be Zhao (whose death I wouldn't pin on Aang as it was clearly the Water Spirit in control at that point) and the wasp-thing from the desert after Appa got kidnapped, which we technically don't know for sure died and either way that action clearly haunted Aang afterward even though it was an animal and not human.
