That was the rule but people absolutely did contact and harass the authors, just like the rule on fandom_wank was never to take wank back to the source but it constantly happened. I got my first fandom death threat way back in 1996!

I think the problem is much more that every fandom space is all over every other fandom space (thanks Tumblr and Twitter) and you can't make community norms happen like you could on LJ or DW. Though I'm also old enough to remember usenet and mailing lists, and boy howdy was there a lot of bullying and exclusion happening on mailing lists.
