Mushrooms, can't stand the texture or taste of them and if they're in a meal it will affect the flavor enough that I probably won't be able to eat it.

Raisins ruin everything they touch, everything. And squash, I hate squash with the burning intensity of a thousand suns

Absolutely, about raisins. I hate the texture of raisins and would be fine picking them out, but they also bleed their flavor into everything else.

Depends on the meal, I guess. Though I've found in most cases, it's a lack of ingredients that'll ruin a meal for me, especially in the way of spices.

Olives, mushrooms, and pickles. They all taste gross.

Corn. You can't just add corn on a whim to meals! Corn is surprisingly sweet, and can ruin the flavor of a meal if not properly balanced out.

Mushrooms for the texture, but I don't mind the flavor. Meat for texture and also I'm vegetarian. Eggplant because every time I've eaten it, I've enjoyed it or at least thought it was okay and then thrown it up, so I don't eat it anymore.

Garlic. I seem to have very strong garlic receptors and everything tastes of garlic for days. Likewise spring onions.

Marrow. It's the cotton-wool texture.

Pickle. Whenever there's pickle in something, it totally dominates.

Also, this one isn't a specific ingredient per se, but there's a specific kind of dirty grease taste that grosses me right out and will completely ruin a dish for me. The worst offender seems to be certain types of sausage, but any kind of meat can produce this specific grease taste. I wish I knew what it was that caused the grease to taste this particular way, so I could avoid it more effectively, but I've never been able to figure it out.

Could it be lard? That's pretty distinctive and kinda gross.

Rancid fat, maybe? (Blech, yuck.)

Avocado, in sandwiches and salads. I just can't take the texture, it makes me retch.
Sadly, I can't handle hot spices - anything more intense than sweet paprika, and I'm coughing and gagging and it's just not a good time.

Raisins in all sweet foods like baked goods and desserts - I'm okay (though not a huge fan) with them in savoury dishes like middle eastern or Indian dishes though.

As soon as anise or Sichuan pepper is in a dish I can't eat it.

Tomatoes, depending on the use. I taste Sour quite strongly, so what is pleasantly tangy for most cooks is Overpoweringly Thin and Sour to me. (They can be nice when used with extreme moderation, though.)

Ruin a meal? Nothing short of putrefaction. There are some foods I don't like but I can either pick them out or suck it up for a meal.
