It was primarily used in anime and manga fandoms, the term came from a hentai called Cream Lemon.

Hahah, amusingly I found out that bit of trivia from a guy I dated who, when he first heard it being used, did not realize it came from there and thought it was a reference to Led Zeppelin's "The Lemon Song". Which, well, that's sort of a logical train of thought, but someone had corrected him. XD
