I literally have no bone in this fight, but I'd want you to consider the option that to some people, gender might be far less important to their identity than you think (even in the real world).

Then taking that in-universe for a non-human character who is very old and gets "reborn" into a new shape every now and then - they must have a very different relationship to their body anyway. Suddenly being a different age/height and suddenly having a different face would likely be equally alienating, yes the Doctor generally shrugs it off.

While I generally agree in the sense that I prefer an original female character over genderswapping a male one - I don't think the reasoning for it is sound in this case.
