If by "gender self identity" you actually meant gender self identity, then yeah, that is transphobic. If what you actually meant was that you don't believe core gender identity changes over time (i.e. one does not become trans or become NB after having previously been cis), and that the doctor was never implied to be NB for the first several decades of the show's run, therefore you find it jarring and retcon-y that they're being put forward as NB now...fair.

Highly debatable, certainly, but understandable as an opinion someone could have.
