No paragraph breaks, that's the biggest one, or dialogue being dumped into a paragraph without having a new line break. I can mentally edit iffy grammar/spelling in my head or a dialogue choice I think is OOC or what have you, but if a fic is just a wall of text I can't focus on it and I check out.

Dom/sub just being used as top/bottom, which is most D/s I come across and why I find 90% of it ungodly boring.
