The details never get discussed in that depth, but I assume the mention of repeated IVF treatments are about Martha's fertility issues. Since making the secret, I've finished the series and have a lot of mixed feelings. On the one hand, yay for a frank discussion from Humphrey's side of how he feels about not having kids, but I'm not sure the show really explored Martha's ambivalence enough.

It focused mostly on her guilt and self-sacrifice, and coming from a female character, that leaves a bad taste in my mouth because it's so cliche. I would've liked to see her admit that she doesn't have a strong desire for kids herself, and that maybe wanting them for Humphrey's sake isn't very healthy for their longterm relationship or her mental health, and that it's FINE to not want children and also FINE to not want to be noble and put herself through multiple rounds of IVF and miscarriage(s) in pursuit of this goal? IDK. Lots of feelings, like I said.
