The Doctor should have found some way to save Donna from what happened to her. It was such a shitty ending for such an amazing person. Even if she couldn't travel anymore, she should have been able to keep her memories.

Steve Rogers should have died fighting Thanos. Especially since despite killing Tony he still lingers around in later movies.

I don't know what should have happened in Supernatural. But it wasn't the finale.

I meant Tony. :p Though, Thanos might. I don't know what is going on in all those TV shows. From what I see, all kinds of crazy stuff happens.
