I think they're assuming that the person who posted the video (whatever that is) thinks the rest of the US medical system isn't run like a for-profit business, because realizing that it is might make them object to the whole thing as unethical. The commenter is saying "this problem is exponentially bigger than you're making it seem, but you're too wedded to capitalism to accept that."
