I heard it's the other way around. Originally hanahaki was about the flowers as a metaphor for unspoken confession that would be cured if a confession happened, but it changed into a more literal disease that comes from unrequited love.... But my source was Twitter and might have been someone trying to make the trope origins sound less problematic so they could enjoy it with a clear moral conscience.
