With European style stalls (that have walls and doors that extend all the way to the floor), no one can see who is in the stalls. Lock indicators show if a stall is free or not. Likewise, depending on the setup, the stalls don't have to be in one room; the stalls can essentially be miniature rooms. Schell's Brewery in New Ulm, MN actually has that kind of concept, and it's great. I heard zero complaints about the setup when I was there a couple years ago.
