Strictly USA here but had the weird experience of growing up in a household that didn't know or use that word at all, and when I first learned it, I just assumed it was a slang term for female genitalia. And despite this, when I learned it was an insult, it didn't occur to me it wasn't gender neutral. It wasn't until adulthood I found out it was considered derogatory towards women.

To this day, the word itself doesn't bother me, but I know what you mean about needing to remind oneself about how others might feel offended or even threatened. (Though to be fair, I don't use it on random strangers and I don't use it on people I know unless it's some kind of inside joke between us. And I despise anyone who uses it specifically to demean women.)
