Also American. I feel you, pal. Gotta spend money to get a doctor's excuse for a job that won't pay sick days and is the very reason you're being made to bring a doctor's excuse in the first place, basically punishing you financially, TWICE, for getting sick and missing work. But I guess anything else would be a communism, so. Usa, I suppose.

Yup. In my mind, the note requirement might make sense if you are allowed unlimited amount of sick days. But we only get 5 sick days (and 10 vacation days). So why do they care so much if I have to use up two of them together and then later in the year have to use vacation days for sick days?
