There's no tests for IBS. It's diagnosed by ellimination. You can just try Omeprazole 20mg for gastritis. And test stool for blood to eliminate anything seriously wrong. Letuce is a weird food for IBS as usually fresh produce is hard on stomach. Baby gluten and lactose free food is usually the safest.

You can also try Trimebutine that is specifically for IBS. That med is pretty safe. Omeprazole is not recommended 2 weeks before endoscopy or if you gonna test for SBO. Also should be stopped gradually. Trimebutane can be taken or stopped whenever, but best to follow directions and be consistant. The lower stomach pain can be from large intenstine too, btw. Also some chamomile tea.

Can it be food poisoning? Then sorbents or some antibiotics.

to add to that blood can be due to annal fissure so don't panic if it's positive.

So I only landed on IBS because at the start it was following that pattern (apples and beans were the first foods to go) but now it just seems random. I can tolerate small servings of yogurt but no other dairy, and iceburg lettuce but no other lettuces. I used to be able to stomach carrots but those are out now too. I've tried all sorts of gas, pro/prebiotic, stomach treatments and nothing seems to work.

And not to get too tmi but it is not exactly constipation or any obvious gas, just intense intense stabbing pain in my lower abdomen that happens about 1-3hrs after eating. There is never passing gas or a bowel movement, just pain until the pain stops - usually 10-24hrs. When I say tests basically I want a colonoscopy and an ultrasound.

Sounds like it could be a number of things, from diverticulitis to MCAS - I'm so sorry you're going through this.

IBS can be without obvious gas - just a feeling that someone kicked you in the stomach. Aanyway, colonoscopy is a good idea, especially at 40+ age. Ultrasaund is so basic that I can't believe it's not free in USA. Good luck. Maybe try asking on some medical forums.

I'm just an IBS on-and-off person. Another thing to consider - it's kinda hard to stick to self prescribed treatments. And even for someone with unlimited funds and time like me, IBS is not exactly easily diagnosed. Doctors just kinda strike out other illnesses.
